About cardiac rehab

Cardiac rehabilitation programs, also known as cardiac rehab, are a structured way to bring together all the things you need to do to keep your heart healthy after a diagnosis and surgery or procedure.

Cardiac rehabilitation programs include education on your heart, medications, healthy eating, mental health and wellbeing, managing risk factors, and returning to everyday life. They can also support you to get back to exercise under the guidance of a specialised team.

Cardiac rehabilitation programs support your recovery from a recent diagnosis and surgery or procedure as you adjust to learning to live with a heart condition. People who attend a cardiac rehabilitation program have improved health and wellbeing and reduce their risk of having heart problems in the future.

Some programs also cater for people living with heart failure, those who have had a pacemaker or implantable cardioverter defibrillator (ICD), and who are living with a heart rhythm problem such as atrial fibrillation.

Our cardiac services directory includes a list of cardiac rehabilitation programs operating across Australia, with descriptions of how the program is run, when they run, any costs and referral/contact details.

Please note some programs included are exercise only – so that they are best to attend after you have completed a Phase 2 outpatient program.

Statewide telehealth cardiac rehabilitation programs

Some states and private health insurers operate telehealth-based cardiac rehabilitation programs. These programs use phone calls and video conferencing to deliver their service. This means that you can access from your own home. For a list of these programs please see here:

Description: Cardihab is a clinically validated nationwide telehealth program. Cardiac rehabilitation telehealth is where rehabilitation services are provided over the phone or by video conferencing. The list below includes some major telehealth services that are available across the nation.

Description: Medibank Heart Health at Home is a phone-based cardiac rehabilitation program that runs over 6 to 8 weeks and you can participate no matter where you live in Australia (eligibility criteria may apply).

Description: The Country Access to Cardiac Health (CATCH) program has been developed to improve access to cardiac rehabilitation and secondary prevention programs (Heart Health) for people living in country South Australia by providing a range of different access options.

Description: Virtual nurse-led service to support people with chronic heart failure at home, using remote monitoring. The program includes virtual consultations with specialist nurses and allied health professionals to help patients manage their medication, diet, exercise and mental health in partnership with their treating doctors. The service aims to educate patients on how to manage and monitor their condition, with aim of reducing hospitalisations and improving quality of life.

Description: The cardiac rehabilitation program is for St Lukes members who have recently been to a hospital for a heart condition or surgery, or have upcoming heart surgery. The program runs for 7 weeks and is delivered by Honeysuckle health, on behalf of St Lukes. The program is designed to support members as they recover, improving understanding of their condition and adjustment to their diagnosis.

Improving the quality of cardiac rehabilitation

The Heart Foundation is working closely with SOLVE CHD and ACRA to improve the availability and quality of cardiac rehabilitation programs across Australia.

On the directory you will see some programs have ACRA/ICCPR Provider Certification status. These providers have completed additional training through the ICCPR International Council of Cardiac Prevention and Rehabilitation, supported by ACRA.

Solve-CHD

Solving the long-standing evidence-practice gap associated with cardiac rehabilitation and secondary prevention of coronary heart disease

SOLVE-CHD is a NHMRC Synergy Grant, a multidisciplinary research team of researchers and clinicians with allied health, nursing, cardiology, public health, health economics and psychology backgrounds. Our goal is to transform post-discharge secondary prevention and reduce the burden of heart disease by decreasing deaths, hospitalisations and costs via a program of work that integrates data, technology, partnerships and capacity building.

The Australian Cardiovascular Health and Rehabilitation Association (ACRA) is the peak body which provides support and advocacy for multidisciplinary health professionals to deliver evidence-based best practice across the continuum of cardiovascular care. ACRA’s aim is to empower health professionals to achieve optimal and equitable outcomes for all affected by cardiovascular disease.
