A list of programs to help patients manage their CVD risk

There are a number of programs which support long-term management of CVD risk.

Personal Walking Plans

A free six-week Personal Walking Plan for patients.

Heart Foundation Walking

Information about networks of free community walking groups. All walks are led by volunteers. The location of the closest groups can be found by entering your postcode into the website, or by phone.

Quitline

A confidential telephone information and support service to help patients quit smoking. Quitline counsellors provide tailored support to suit each patient’s needs.

Life! (VIC only)

A free Victorian lifestyle modification program that helps patients reduce their risk of type 2 diabetes, heart disease and stroke. Run by expert health professionals, the program is delivered as a group course or a telephone health coaching service.

My health for life (QLD only)

An evidence-based behaviour modification program that supports Queenslanders at high risk of developing heart disease, diabetes or stroke. The program aims to support participants to set goals and action healthy lifestyle changes to minimise preventable health risks.

Get Healthy SA (SA only)

Free personal telephone based information and health coaching service. The Wellbeing SA service provides the expertise and motivation to help you reach your health goals. Available Monday – Friday from 8am- 8pm.

Get Healthy NSW (NSW only)

Free personal telephone based information and health coaching service. The NSW Health service can help provide you with personalised support and motivation you need to reach your own healthy lifestyle goals. Available Monday – Friday from 8am- 8pm.

It’s the Take Control - Live well Program (ACT only)

A 3-week group program that gives patients the skills and confidence to take control of their chronic condition.

HealthPathways

An online health information portal for general practice staff to use at the point of care. It provides information on how to assess and manage medical conditions, and how to refer patients to local specialists and services in the timeliest way. First-time users will need to request access.

Tip icon Tip: If you find that your patient has had a CVD event in the last 12 months, you can refer them to the Heart Foundation MyHeart MyLife program.
