My Heart, My Life program and support resources

The My Heart, My Life Program is currently being redesigned!

Our team at the Heart Foundation is working hard to redesign our flagship support program for people living with heart disease, and their support carers. The SMS component is currently undergoing a refresh and will be unavailable for anyone who enrols after June 30.

During this time, you are still able to sign up to the My Heart, My Life program and receive:

  • A printed booklet to help you stay healthy into the future, that will be posted to you,
  • Access to peer support via the My Heart, My Life Facebook Community,
  • Regular emails including links to recipes and walking guides,
  • Access the most up to date information via the Heart Foundation website.
This program is specifically designed for people living with heart disease, and their support carers.
