Identifying eligible patients

Information and links to help generate lists of eligible patents

There are criteria to be met for a patient to be eligible for a Heart Health Check. The patient must:

  • be 45 years or over (30 years or over for Aboriginal and/or Torres Strait Islander Peoples)
  • have no history of CVD, and
  • have not had another health assessment in the last 12 months e.g. MBS items 701, 703, 705,  707 or 715.

Identifying eligible patients

Pen CS – using CAT4

For CAT4 users, the following suite of recipes has been developed to help you identify patients who are eligible for a Heart Health Check. Follow the instructions here in the recipe to extract a specific list.

  • Identify patients eligible for the Heart Health Check who are at high risk of CVD due to hypertension or hyperlipidaemia. 
  • Identify at-risk patients who have a non-CVD chronic condition and are eligible for both a Heart Health Check and a GP Chronic Disease Management Plan.
  • Identify patients eligible for the Heart Health Check who are at low, moderate, or high risk of CVD and have all data recorded for an absolute CVD risk assessment.
  • Identify patients eligible for the Heart Health Check who have missing data for an absolute CVD risk assessment.
  • Identify female patients who are eligible for the Heart Health Check and have had a diagnosis of gestational diabetes.

Pen CS – using cardiovascular event risk report from CAT4

The CV Event Risk graph displays your data as the % risk of a cardiovascular event in the next five years. Click here for more information.

Pen CS – using the clinical information system (CIS) calculated cardiovascular event risk

The report, which shows CV event risk calculated by the built-in calculator in MedicalDirector and Best Practice, can be found here.

Pen CS – using Topbar

In Topbar, there is an MBS app which can give you prompts if a patient is eligible for an MBS item programmed into the app such as 699 or 177. This can help you identify patients opportunistically.

Polar offers the following walkthroughs to help you identify patients eligible for the Heart Health Check:

POLAR – identifying patients eligible for the Heart Health Check at point of care

POLAR Walrus is a tool that allows practices to view single patient analytics and immediate insights relevant to the patient in front of you.

It provides MBS eligibility indicators for patients at the point of care, including the Heart Health Check item. More information, including installation and use, is available here.

POLAR – identifying patients at risk of developing CVD

POLAR’s CVD report allows you to identify patients who are at risk of CVD. This report (based on the Australian Absolute CVD risk calculation), reports on individual risk factors and calls out old/missing indicators such as pathology or other measurements.
