My health for life is a free healthy lifestyle program helping Queenslanders to live and age well.
The program supports people to get their health back on track, particularly those who are at high risk of developing heart disease, type 2 diabetes and stroke. With the help of a professional health coach, participants learn about the benefits of a healthy lifestyle while working towards achieving their own health goals.
Funded by the Queensland Government and managed by the Healthier Queensland Alliance, the program provides six structured sessions over six months, covering topics such as quality sleep, stress management, healthy eating and physical activity.
My health for life works in partnership with general practice and is a practical extension to the advice and care provided by GPs and nurses to their patients.
As a member of the Healthier Queensland Alliance, the Heart Foundation is a proud partner and supporter of the Queensland My health for life program.
