Do pacemakers improve outcomes in patients with high-risk unexplained blackouts?

Approximately half of patients who present to hospital with a blackout are discharged without a clear diagnosis. Pacemakers may help prevent these episodes of unexplained blackout, however identifying which patients will benefit is challenging. Our study will look at using a newly developed risk score, 'the DROP score', to work out which patients will benefit from pacing. We will recruit patients who have had an unexplained blackout and high DROP score and they will be randomized to receive upfront implantation of a pacemaker or conservative care without direct pacemaker implant. Patients will be followed up for 12 months after enrollment. Our research will aims to evaluate whether early pacemaker implantation is the safest and most efficient way to manage these patients.
