A personalised, behaviour change digital health program (Gro-AUS) for improving heart health

Clinicians lack effective, scalable, personalised, remotely delivered lifestyle programs to support patient self-management to improve cardiovascular (CV) health. ‘Gro Health’ is an international, co-designed, evidence-based, behaviour change digital health platform that promotes high culturally and linguistically diverse engagement and effectively treats chronic conditions including obesity and cardiometabolic diseases. The Gro Health program has recently been localised for use in Australia (Gro-AUS). This project will assess the feasibility and effectiveness of delivering Gro-AUS through the Australian healthcare system to provide clinicians with a scalable patient self-management solution that can be prescribed to patients with increased CV disease risk as a part of their chronic disease management plan. This will result in a reduction of CVD risk complications and healthcare costs with direct relevance for patients and policymakers to fund support for cost-effective, evidence-based, scalable patient self-management programs.
