Nutrition Resources for Practice and Patient Care

To help your patients to adopt heart healthy eating patterns, you can encourage them how to make small changes that they can continue long term. We have developed these resources for health professionals to translate the latest nutrition evidence for use in your everyday practice.

Sodium and salt converter 

Australians are eating almost double the recommended amount of salt. To help your patients reduce the amount of salt in their diet, this tool will help them to read food labels and convert the sodium listed on food products to grams of salt, or vice versa. 


Eating well to protect your heart (2020) 

This resource outlines what a healthy eating pattern looks like. It also includes tips for healthier food choices and ways to prepare healthy meals. 

Eating well with high blood pressure (2020) 
This resource provides practical healthy eating advice for patients living with high blood pressure or hypertension. It includes ways to reduce salt in their diet. It also includes ideas for adding flavour to cooking using herbs and spices instead of salt.  
Eating well for a healthy weight (2020) 
Eating an unhealthy diet can increase your patient’s risk of developing coronary heart disease. Being above a healthy weight can also increase your risk. We developed this guide to help people achieve and maintain a healthy weight. 

It contains tips for following a healthy eating pattern by making healthier food choices and ideas for preparing meals at home. It also includes useful tools that your patient can use to maintain a healthy weight. These tools include how to set weight loss goals, measure waist circumference and calculate Body Mass Index (BMI). 
Heart Healthy Catering - Quick Reference Guide 2020 
This is a quick reference guide containing a selection of heart healthy menus. Providing heart healthy menu choices to your patients helps raise awareness about the Heart Foundation’s Heart Healthy Eating Patterns. It also creates the opportunity to promote heart healthy foods and nutrition messages.  

Sources of marine and plant-based omega-3 (2015) 


The Heart Foundation recommends that all Australians should aim to eat two to three serves of fish (including oily fish) per week as part of a heart healthy diet. This resource contains a handy guide to the amount of marine-sourced omega-3s (EPA and DHA) in the fish and seafood commonly available in Australia. There is also a list of the amount of plant-sourced omega-3s (ALA) contained in commonly consumed foods in Australia.
