Heart failure resources for patients

Educate your patients on everything they need to know about heart failure and managing their condition.

We have developed these resources are to help educate your patients with heart failure about their condition, how to recognise signs and symptoms, information about medications and things that can make them feel better.

Resources include our new heart failure video series for patients and our Living Well with Heart Failure booklet.
Three self-management tools are also available below to print and give to your heart failure patients.  

Heart failure video series 

This video series is for patients and provides information about medicines for heart failure, things to do to make you feel better, heart failure and physical activity and what to do when you feel sick.

Heart failure booklet 

The Living Well with Heart Failure booklet (PDF) contains information for patients about what heart failure is, how it feels, and what to do if you’re feeling sick. The booklet also advises those with heart failure on ways to feel better each day, as well as providing care reminders, and an emergency plan in case symptoms get worse.  

Measuring your blood pressure at home 

High blood pressure is a risk factor for heart failure. Measuring your blood pressure at home (PDF) provides information and tips for patients on measuring blood pressure at home. It also contains information on how to get an accurate blood pressure reading and advice on purchasing blood pressure monitors.  

Take Action in 24 Hours 

There are several signs that will indicate when patients need help managing their heart failure, such as weight loss or weight gain, difficulty breathing and dizziness. Providing your patients with a copy of Take Action In 24 Hours (PDF) will alert them to get help when experiencing any of these symptoms.  

My fluid plan 

Fluid intake plays a role in heart failure management but it can be hard for some patients to self-manage. Providing your patients with a copy of My fluid plan (PDF) can help them understand how much fluid they can have each day, as well as tracking their intake.
