Heart Health Check Toolkit – baseline evaluation survey

Contest is open to Australian residents who have registered to receive the Heart Health Check Toolkit and who complete the Heart Health Check Toolkit baseline evaluation survey.

Participation in this promotion is subject to the Heart Health Check Toolkit – baseline evaluation survey Details of Participation.

Any entrant found to be creating multiple social media accounts (e.g. multiple Twitter accounts) in order to submit more entries will be disqualified.

Details of Participation

Information on how to enter forms part of these Details of Participation and entry is deemed as acceptance of them. No purchase is necessary to enter the contest or redeem prizes. Contest commences on 8 February 2021 and entries close at 11.59pm on 28 February 2021. Entries not received by that time & date or not completed in accordance with the Details of Participation will not be eligible to win.

Prize

Three Coles Myer vouchers (valued at $200 each). Total prize pool is valued at $600. If any prize is unavailable, the promoter reserves the right to substitute another item for the prize, in its sole discretion, of equal or higher value. All prize values are correct at the time of printing. Prizes are not transferable or redeemable for cash unless otherwise indicated.

Who can enter

Contest is open to Australian residents who have registered to receive the Heart Health Check Toolkit and who complete the Heart Health Check Toolkit baseline evaluation survey. Employees and volunteers of the promoter and their immediate family members ARE NOT eligible to enter.

How to enter

Contest can be entered by completing the survey and filling in your contact details at the end of the survey. Only one entry per person is permitted.

Prize Draw

The odds of winning are based on the actual number of entries received. Neither the promoter nor any of their agencies are responsible for lost, late, damaged, misdirected, incomplete or ineligible entries. The Promoter’s decision is final, and no correspondence will be entered into. All prize/s are guaranteed to be given away by random drawing from all methods of entry of this contest. All prize draws and redraws will take place at 850 Collins Street, Docklands, Vic 3008 at 9am on the 1 March 2021.

Winners will be notified by phone or email where possible.
