Triple therapy prevention of Recurrent Intracerebral Disease EveNts Trial (TRIDENT) – Cognitive sub-study

Professor Sharon Naismith, Institution: University of Sydney

2017 Vanguard Grant

Years funded: 2018-2020

Cardiovascular (CV) disease is a major contributor to diseases of the brain and mind.

In addition to contributing 30-50% of the risk of dementia, CV risk factors including: hypertension, smoking, obesity, diet and physical inactivity which have been shown to account for more than 80% of the global risk of stroke.

Acute intracerebral haemorrhage (ICH) is the most serious and least treatable form of stroke. Alarming new evidence suggests that within four-years approximately one quarter of ICH survivors develop new-onset dementia.

Importantly blood pressure (BP) lowering can reduce the risk of recurrent ICH and other serious CV events, however many ICH survivors have inadequate BP control increasing their risk of dementia.
This study will evaluate the efficacy of a triple dose BP lowering pill for ICH against detailed neuropsychological outcomes.

This opportunistic pilot will be embedded within a large international NHRMC funded clinical trial: Triple therapy prevention of Recurrent Intracerebral Disease Events Trial (TRIDENT).

This is the first ever study of triple dose BP lowering for ICH focused on preventing dementia. The results will provide robust evidence with regard to targeting CVD as a way to prevent dementia and have immediate research translation potential.
