Total Cardiac Care - A randomised trial of a smartphone application and care model

The overall aim of this project is to trial the use of a smartphone application (app) and associated cardiovascular care model. The app, entitled Total Cardiac Care, has multiple functions. These include wireless connectivity to a blood pressure machine and weighing scales, real-time activity monitoring and notifications (based on the Heart Foundation Managing my Heart Health document) delivered to the smartphone to educate and motivate participants.

The data collected by the app will be monitored by the research team, and if variances from the parameters are detected, such as a significant change in weight, blood pressure or activity level, then the participant will be contacted and if appropriate, attend their local doctor, cardiologist or the community-facing hospital cardiac assessment clinic. Participants will be recruited prior to discharge from hospital after an admission for a heart attack, unstable chest pain or heart failure. Each participant will be recruited for a six month period.

The expected outcome is a reduction in hospital readmission rates. This will in turn translate into a reduction in healthcare costs, while also lessening the demand on inpatient cardiology services as well as the Emergency Department. Other endpoints to be analysed include medication compliance, quality of life, weight, blood pressure, cholesterol levels and cost analysis.
