The Colchicine to prevent Peri-Operative Complications Trial (The Co-POC Trial)

The most common complication after cardiac surgery is myocardial injury, or damage to the heart muscle, which has been associated with much poorer short and long-term survival. The primary aim of this pilot trial is to establish feasibility, tolerability, and safety of low-dose colchicine after cardiac surgery. In addition, we will obtain vital data regarding the efficacy of colchicine, in terms of its ability to reduce myocardial injury in this setting. The investigators on this application are contributing to a major international observational cohort study - The Vascular events In Surgery Patients Cohort Evaluation – Cardiac Surgery (VISION Cardiac Surgery) - that will recruit 15,000 patients from fifteen centres worldwide undergoing adult cardiac surgery. The primary aim of VISION Cardiac Surgery is to clarify the relationship between myocardial injury, detected using a high sensitivity troponin I assay, and 30-day and 1 year survival. One thousand patients will be recruited from Royal Prince Alfred Hospital (RPAH), Sydney. This project is to fund a local, nested, randomised vanguard trial of colchicine, a unique anti-inflammatory agent, versus matched placebo in 204 patients recruited into VISION Cardiac Surgery at RPAH. If this vanguard phase confirms feasibility and suggests efficacy, we will seek additional funding to complete the definitive trial.
