Strategies to optimise cardio-metabolic health

The aim of this proposal is to undertake research and develop tangible resources to enhance the health of individuals with or at risk of heart disease and diabetes (cardio-metabolic disease) by helping them live longer and more fulfilling lives out of hospital. Targeting two of Australia's health priority areas, this research will assess the benefits of nurse-led management programs (including nurse run regional clinics) and the use of decision support tools across the spectrum of cardio-metabolic disease. It is expected that this research will yield additional knowledge to improve the outcomes, quality and (cost) effectiveness of care, with the aim that what is learned in the clinical research setting can be put into routine clinical practice to optimise patient care and management. If deemed positive and cost effective, more nurse-led clinics may be set-up in regional Australia where there are higher levels of risk. There is also the potential for this research to facilitate capacity building beyond traditional models of care. This research may provide practical blue-print(s) for health services to implement strategies to better manage thousands of Australians each year with cardio-metabolic disease.
