Impact of early coronary angiography in patients post cardiac arrest without ST elevation on ECG

Heart disease is the leading cause of death both in Australia and around the world. Out of hospital cardiac arrest (OHCA) accounts for over half of all fatal cardiac events. Outcome among patients following OHCA is generally poor with only just over half having successful restoration of a circulating rhythm, and 10% surviving to discharge from hospital. This subsequent morbidity and mortality is due mainly do cardiac and brain dysfunction, and systemic inflammation that follows a prolonged period of poor circulation and organ perfusion. It has been previously shown that optimizing post-resuscitation care such is associated with improved patient outcomes. However the usefulness of emergent coronary angiography as a standard of care for all resuscitated post-OHCA patients is controversial, particularly in the absence of ECG changes suggestive of coronary disease, although the potential for early revascularization in helping cardiac recovery and improving chances of survival are well recognised. This research aims to assess the safety and effectiveness of emergent coronary angiography in a randomized population of post-OHCA patients without ECG changes. Findings are expected to help guide management of this challenging patient group in the future.
