Supporting Young Hearts

Advice from young people living with a heart condition.

The Supporting Young Hearts program provides opportunities for young people to connect with each other, share stories and learn about how to manage their heart condition.

An advisory group of young people living with heart conditions help design, support, and guide the direction of the program. 

The program helps to provide life stage specific connection, support, and information in the following ways:
ConnectionOpportunities to meet and connect with others who are living with similar conditions and have shared experiences.
SupportProviding support for their emotional health, managing their condition and recovery.
Information Customised information to help manage their heart condition and emotional health and wellbeing.

This forum for younger people aged 18-45 years living with heart conditions provided information on managing emotional health and wellbeing. It aims to provide patients with the right tools to overcome emotional challenges and thrive. 

Patient stories

Young Australian men and women share their personal stories of diagnosis, recovery and coming to terms with the physical and emotional challenges of living with a heart condition.

Information pack

The Heart Foundation has produced a pack of information especially for young people dealing with heart disease.

The pack can be delivered via an email or SMS link and contains:
  • A collection of videos of six young people telling their stories of living with a heart condition from diagnosis, surgeries, recovery and how they keep on track with lifestyle changes.
  • Information sheets that look at blood pressure, cardiac rehabilitation, anxiety and depression and more
  • Animated videos with tips on medications, going back to work, quitting smoking and preparing for hospital
  • Tools and resources including key websites for more information.
