Heart Foundation Practice Nurse Survey 2013
The Heart Foundation recognises the vital role that general practice plays in the prevention, diagnosis and ongoing management of cardiovascular disease. As a Practice Nurse, we would like to invite you to participate in a survey that we are conducting across Australia.
About the survey
This survey has been designed to collect feedback from Practice Nurses about their awareness and use of Heart Foundation materials and key messages, as well as gaining a better understanding of their interaction with clients and the practices they employ.
Collecting this information will assist us in better understanding General Practice, and help us improve our communications to assist health professionals in diagnosing and supporting patients with heart disease.
How to complete the survey
To access the survey, you will need to enter the password provided in the invitation letter that you received. You will need to complete the survey in a single sitting.
The survey should take about 10 minutes to complete.
