New: Multidisciplinary Care For People With Chronic Heart Failure

The Heart Foundation’s new paper "Multidisciplinary care for people with chronic heart failure" is now available (see 'Resources' below).

This paper complements the Heart Foundation and Cardiac Society of Australia and New Zealand’s "Guidelines for the prevention, detection and management of chronic heart failure in Australia, 2006".

It provides principles and recommendations to help health professionals and policy makers establish and maintain best-practice chronic heart failure (CHF) management programs in Australia. It also aims to improve clinical outcomes and quality of life for people with CHF.

Patient Self-care in Chronic Heart Failure
People with chronic heart failure (CHF) who can effectively manage their symptoms through lifestyle modification and adherence to complex medicine regimens will experience less hospitalisations and other adverse events.

Information Sheets for Patients and Carers Now Available in Other Languages
Our information sheet for people with heart failure and their carers:

  • includes information on how to minimise the symptoms of heart failure and stay healthy
  • has an 'action plan', listing the important things to do every day and when to call a health professional or an ambulance.

It is based on our booklet "Living well with chronic heart failure" and is now available in English and 16 other languages (see 'Resources' below).

Living Well With Chronic Heart Failure Booklet
The booklet "Living well with chronic heart failure" is designed to help people with chronic heart failure better understand and manage their condition. It also makes it easier for health professionals to discuss symptom management, lifestyle changes, medicines, other treatments and other issues.

Guidelines
Guidelines for the prevention, detection and management of chronic heart failure (CHF) were developed by the Heart Foundation and the Cardiac Society of Australia and New Zealand.

The guidelines were developed for GPs, cardiologists, nurses and other health professionals involved in the care of patients with CHF in Australia. They aim to help health professionals improve the management of patients with CHF and ultimately, health outcomes and quality of life.

These guidelines have been endorsed by the Royal Australian College of General Practitioners (RACGP), Heart Support Australia (HSA), Royal College of Nursing Australia (RCNA), Internal Medicine Society of Australia and New Zealand (IMSANZ) and Australian Cardiac Rehabilitation Association (ACRA).
