Terms & Conditions - Area Coordinators
The Heart Foundation acknowledges and accepts its responsibilities to provide adequate training, equipment, resources, services and support to enable Heart Foundation Walking Area Coordinators to fulfil their role with the Heart Foundation Walking initiative.
I, agree to act as an Area Coordinator for Heart Foundation Walking in accordance
with the directions given by the Heart Foundation.
Upon receipt of the Area Coordinator Implementation Guide, I agree to read it and contact the Heart Foundation if I have any questions about its contents.
I agree to comply with the Area Coordinator Implementation Guide and I will not undertake any activity, other than outlined in the Area Coordinator Implementation Guide, including activities which involve or implies involvement with or of the Heart Foundation, or the Funding Partners except with the prior written authority from the Heart Foundation.
I agree to acknowledge the Heart Foundation and the Program Funding Partners when promoting Heart Foundation Walking.
I agree to use the following statement to describe Heart Foundation Walking when promoting the initiative: "Heart Foundation Walking is Australia's largest network of free community based walking groups coordinated locally by _________________(insert your organisation, council or health service name) ".
I agree to comply with the Heart Foundation's Privacy Policy* as summarised in the Area Coordinator Implementation Guide, and will not disclose or use personal information for any purpose other than for which it was collected.
I agree to comply with these terms and conditions as may be amended by the Heart Foundation from time to time and acknowledge that the Heart Foundation will display up to date terms and conditions on www.heartfoundation.org.au.
I agree to immediately advise the Heart Foundation in writing if I am no longer employed by my organisation.
I agree to immediately cease to act as an Area Coordinator on receipt of written notice from the Heart Foundation.
